National Health Care Decision DayThursday, April 16, 20158:00-10:00 a.m. at the Hamilton Hy-Vee CafeteriaStaff will be available to help you fill out an Advanced Directive for your medical wishes.
An advance directive is a legal document that tells healthcareproviders who it is that you wish to make medical decisions for you.It also lays out what treatments you would want or do-not wantif you are not able to tell someone, due to an accident or illness.National Healthcare Decisions Day exists to remind all people,regardless of age, the importance of having your wishes in writing.These forms are free and will be available at this event.
